Health forms for summer camp needed this Tuesday 6/27

Hello T215-

Woodruff has requested we mail the health forms 2 weeks in advance of summer camp in order to speed up registration.

We need forms for summer camp that include a doctor’s physical. It would be GREAT to know that everyone has a physicians appointment set up.  Many of you on the list have health forms that are expired.

IF you are on this list, please let me know your appt. is set.

For Woodruff Summer Camp, we need this forms in ASAP because Woodruff expects us to send copies to camp WEEKS IN ADVANCE of our actual stay there, so I am asking everyone to please get the physical forms completed and turned in A.S.A.P.
