TK Healthforms

Good afternoon-

Some of you are turning in your completed ABC healthforms for TK tonight.  I have asked all of you to please come with a photo copied healthform for camp as well as your original form for our Troop Healthform book.

Please also be sure to FOLLOW THE PART B HEALTHFORM INSTRUCTIONS and attach a COPY OF BOTH SIDES OF YOUR INSURANCE CARD to each form as well, most importantly the copies going to camp.

I will be at the meeting only briefly from 650-720 to collect forms and pass a packet off to Mr. Awesome.  If you miss this window, please see Mrs. Pientka to hand her your ORIGINAL for our healthform book, and hand your photocopy to Mr. Awesome (Mr. Leikala) for camp.
